These complex conditions contribute to many difficulties in weight management that require dietary management and additional health support beyond the limited management in traditional healthcare settings. Our goal is to make life-changing GLP-1 medications accessible and affordable for those who need them, helping patients manage their weight and improve their overall health and quality of life.

Who Can Benefit from the LifeFit Medication Initiative?
The LifeFit Medication Initiative is specifically tailored for individuals facing obesity or significant weight-related health challenges that are often associated with underlying conditions, including:
PCOS (Polycystic Ovary Syndrome): A hormonal disorder that can cause weight gain, infertility, and other health issues.
Hormone Imbalances: Conditions that disrupt normal hormone levels, leading to weight gain and difficulty losing weight.

Our Approach

How Patients Can Participate
Eligibility Criteria:
BMI and Health Conditions: Patients must have a BMI of 30 or higher, or a BMI of 27 with comorbid conditions such as PCOS, hormone imbalance, thyroid disorders, or prediabetes.
Medical Diagnosis: Must have a diagnosis of obesity, a weight-related health issue, or related comorbid conditions.
Financial Need: Applicants should provide proof of insurance denial or documentation showing out-of-pocket costs exceeding $75 for GLP-1 medications.
Commitment to Treatment: Patients must agree to comply with the prescribed medication regimen and participate in the PinkLife Diet Program, which supports healthy lifestyle changes.

Application Process
Complete the Online Application: Patients can complete an appointment request to assess eligibility for the program. Schedule here.
Submit Required Documentation: Upload necessary documents, including medical records, proof of BMI, insurance denial letters, and financial information.
Initial Screening: Our team will conduct an initial review to verify eligibility and inform you of next steps.
Detailed Evaluation: If the initial criteria are met, a healthcare professional will review the patient’s medical history and timeframe for treatment and our counselor will assess the patients financial situation.
Approval and Enrollment: Approved patients will be notified and enrolled in the LifeFit Medication Initiative, where they will receive support and access to medications through our partner pharmacies.
Ongoing Monitoring: Patients will be required to register with AdairClinic, where they will receive ongoing monitoring, support, and counseling to help them achieve their health goals for the duration of their program.

Referral Process:
Identify Eligible Patients: Assess your patients for eligibility based on BMI, diagnosed health conditions, and financial need. Patients with obesity, PCOS, hormone imbalances, thyroid disorders, or prediabetes are ideal candidates for this initiative.
Submit Secure Referral Request:
Submit a patient referral request through your EMR or fax to 1-833-449-5032. Include detailed information about your patient’s medical history, diagnosis, and current treatment plan.
Submit Documentation:
Provide necessary documentation to support your referral, such as recent lab results, medical records, and proof of insurance denial.
Patient Application:
Once the referral is submitted, your patient will be contacted to complete their portion of the application and provide additional required documents.
Coordination with Adair Clinic:
If approved, your patient will be registered with Adair Clinic, the assigned clinic for managing LifeFit participants for ongoing monitoring and support throughout the program.
